CDM Table name: OBSERVATION_PERIOD
Occasionally registration end date (regenddate) is NULL. In this case take the practice last collection date (lcd). Otherwise, take the earliest of last collection date and regenddate for the OBSERVATION_PERIOD_END_DATE.
